High Intensity Support

Specialised NDIS care for complex needs

High intensity disability care is specifically for people with complex needs who require additional assistance to accomplish everyday tasks.

Depending on your circumstances, you may require high intensity support rather than standard care.

Our support workers are trained with specialised qualifications and experience to ensure they can understand the nuances of your condition/s and provide care to meet your needs.

Implementation and staff training

The supports are provided in line with our Service Delivery policy, Harm Prevention policy, and Waste Management procedure.

Participants who require support with their health needs must have a health care plan for workers to follow. The plan needs to be written by either a doctor or nurse to ensure proper precautions are taken to mitigate risk for both the person and employee. An approved plan means participants can receive high quality and safe supports.

To ensure our NDIS clients receive the highest level of care, Support Services Queensland follows a strict protocol.

When a high intensity support need is identified (either at time of intake or due to change in circumstance of a participant), the following procedure is implemented:

  1. Client risk profile is updated
  2. Individual risk assessment is completed
  3. Support plan from the treating doctor or registered nurse is saved to the client’s file
  4. Needs are clearly identified to their direct support care team
  5. Staff have relevant training by the author of the support plan OR relevant external training provider
  6. Confirmation of training is to be recorded and managed

Supported Independent Living (SIL)

Empowering Your Independent Life

Supported Independent Living (SIL) is assistance with or supervision of daily tasks to ensure NDIS participants can live as independently as possible in their own homes.

It includes paid support to build skills and increase capacity in a home setting, most commonly used in shared living arrangements.

SIL support approval process

In order to be approved for SIL support funding, you must follow an application process.

STEP 1
Do you have home and living arrangements as a goal in your NDIS plan? If not, we can support you through this process. It may take approx 6 weeks. Please note that it will require a change of circumstance to be lodged with the NDIA.

STEP 2
Once you have Home and Living arrangements as a stated goal in your NDIS plan, you may engage your preferred allied health professional for a capacity assessment to determine eligibility. This can take approx 1-2 months depending on availability.

STEP 3
Once the allied health reports are completed, you may begin searching for, and engaging with, your preferred SIL provider. Refer to our site regarding what things you should consider when searching for a SIL provider.

STEP 4
Your chosen SIL provider will assist with your SIL Submission documents, allied health reports, and an additional change of circumstance to the NDIA. The NDIA will take time to consider your application and if you are eligible for SIL supports. This can take a few weeks or months.

STEP 5
If your application is accepted, the NDIA will approve your SIL funding and issue a new NDIS plan. You may begin planning with your SIL provider for a smooth transition into your new home.

STEP 6
If you are denied SIL support, it is important to request details of the decision making to allow you an opportunity to request a review with additional supporting evidence if appropriate.

Individual Support

Tailored Support for Your Unique Goals

Support Services Queensland has spent many years developing and adapting how best to provide individual support.
We have learned that consistent consultation and flexibility are two fundamental practices for success.

Each request for individual support is different, and to ensure that each participant gets the most out of their funding, creating a schedule of support and establishing clear goals is highly important.

Throughout the intake process, conversations will be had with the participant, their loved ones and any additional stakeholders to gather relevant information. This information will aim to help you to achieve your goals.

Short Term Accommodation

AND RESPITE

Short Term Accommodation (STA) or respite is temporary care of loved ones that you can utilise when you want a holiday or break from your current living situation.

We can utilise STA funding when you are transitioning into a new SIL arrangement and would like a trial period.

Confused about the process or how to apply for STA and respite funding?

  • Support Services Queensland can assist you with planning your stay, including itinerary, activities, meal plan assistance, and more.
  • Our team can provide emergency STA options in times of crisis or hardship
  • Refer to your NDIS plan to determine if you have access to STA funding. You can discuss your STA needs with your local area coordinator or support coordinator.

Individual Living Options (ILO)

alternative ways of supporting and funding living arrangements for NDIS participants

There are lots of choices with ILO. Participants can share their home with friends or housemates. It may be preferred to live in a Host arrangement or on their own with tailored support.

The exploration stage prioritises creating a safe environment considering the participants’ goals, choices and values.

Support Coordination

Expert Guidance for Your NDIS Journey

A support coordinator is a funded point of connection for the participant. Their role includes assisting participants to develop their knowledge and experience as well as assisting them with making connections in their community.

Support Services Queensland strives to provide high quality support coordination to its service users.

Participants (and families) seeking support coordination will be supported to build the participant’s (or family’s) capacity to manage relationships, manage service delivery tasks, live more independently and be included in their community.

Specialist NDIS support coordination

A higher level of support coordination whose situation is more complex and requires specialist support. A specialist support coordinator will assist a participant to manage challenges in their support environment and ensure consistent delivery of service.
